Diabetes
Diabetes
Western Queensland PHN consider diabetes to be one of the top priorities for the region. Diabetes contributes to a significant burden of illness and diabetes-related complications are one of the main reasons for hospitalisation in WQ. In some of our remote communities the prevalence of diabetes is up to 20 per cent. The Western Queensland Diabetes Service Framework sets out WQPHN's vision to optimise care for people living with diabetes in our patch.
People living with diabetes require a consistent and coordinated approach to their care. Click the links below to access resources to support diabetes education and management.
