Client Need:
Integrated Team Care for Aboriginal & Torres Strait Islander people. Social and Emotional Wellbeing Culturally aware holistic care.
Ages – all ages
Service Details:
Provide culturally and clinically integrated social and emotional well-being service, within a person-centred stepped care approach, which is accessible, affordable, culturally responsive to Aboriginal and Torres Strait Islander Peoples.
Also targeting Aboriginal and Torres Strait Islander Peoples with co-morbidity conditions - mental health and chronic disease.
Support provided by brief interventions, counselling, initiating and managing planned treatment options, consultation and referral to other agencies or crisis management.
How to access:
The following referrals are accepted under this program:- Local Aboriginal Medical Services
- Self-referral
- Family/carer referral
- Allied Health professional referral.
Stay Strong application used to map client’s social and emotional wellbeing journey.
No Mental Health Treatment Plan required.
