The unique challenges of the remote catchment and poor health status of Western Queenslanders presents a compelling case for collaboration and partnership to bolster the capacity of the WQPHN to positively influence and transform the systems and services that support better primary health care outcomes.
Through partnerships with the Western Queensland HHSs and the ACCHO sector, adopt whole of system co-commissioning across the region.
Prioritise primary health care delivered as close to people’s homes as possible to reduce avoidable emergency department attendances and hospital admissions.
Strengthen organisational linkages to enable integrated care.
Regionally plan allied health and medical specialist services Advocate for better support for transport, accommodation and coordinated scheduling when patient travel outside the region is necessary.
