Western Queensland PHN 2016 Health Needs Assessment Technical Paper
The Western Queensland PHN is an independent not for profit company formed by the three Western Queensland Hospital and Health Services (HHSs) to create an entity to foster partnerships with funders and providers to improve primary healthcare service delivery to the people of Western Queensland. Under this Commonwealth initiative a major goal is to work to improve the integration of primary care services and support more seamless health care in communities across Western Queensland with the key objectives being to:
- Increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
- Improve coordination of care to ensure patients receive the right care in the right place at the right time.
